For citizen complaints/concerns, you may simply call or email ( the Administrative Assistant/Public Records Officer at  419-822-4371 or you can print the form linked below, fill it out, and mail it to us or drop it off to the Administration building Swancreek Township, 5565 Co. Rd. D, Delta, Ohio 43515.
We also have, for your convenience, a drop box located at our front entrance.
